RESIDENT CARE PROGRAMS

Facility Program: Fall Prevention

Empowering Healthcare Staff to Reduce Fall Risk

The primary goal of this program is to educate healthcare staff on the significance of fall prevention, including understanding the burden of falls, identifying fall risk factors, and implementing best practices to reduce fall incidents within their facility.

Facility Program: AFHS

Age-Friendly Health Systems Recognition

At least 1 staff person can lead the Age-Friendly Health System Recognition team from your executive leadership, administration staff, or clinicians. Participants will gain an understanding of the importance of providing an AFHS to all older adults in your facility, along with how to align care with each older adult's health outcome goals and care preferences.

Facility Program: Dementia Care

Dementia CareForce

 Gain practical hands-on skills to navigate challenging behaviors presented in people living with dementia. This program focuses on understanding unmet needs, problem-solving tools, and utilizing huddles for enhanced communication.

Staff Program: Clinical Practices

Diabetes Care for Older Adults Certificate

This certificate program for nursing home clinicians caring for older adults with diabetes has multiple goals to enhance patient care and staff competency/knowledge.

Staff Program: Virtual Sessions

Long-Term Care RISE ECHO

This series seeks to empower facility administration and staff to improve health outcomes for residents facing challenges related to age-friendly care, by providing an innovative and evidence-based approach to increasing high-quality care for residents in the communities where they live. 

Leadership Program: Fall Prevention

The Falls Management Program

Leaders meet one time virtually to learn how to improve fall care processes and outcomes through educational and quality improvement tools. While not all falls and injuries can be prevented, the program emphasizes the importance of a systematic process of assessment, intervention, and monitoring to minimize fall risk.

Staff Program: Behavioral Heath

COE-NF Behavioral Health Resources 

Established by the Substance Abuse and Mental Health Services Administration (SAMHSA), in partnership with the Centers for Medicare and Medicaid Services (CMS), the COE-NF offers Certified Medicare and Medicaid Nursing Facility Staff a centralized resource hub with easy access to trainings, technical assistance and additional resources, at no cost.

Staff Program: Behavioral Health 

PPI De-Escalation Training

This training covers the following areas: 

  • Building Emotional Resiliency (tips and skills to build self-care and self-awareness)
  • Preparing for Acuity
  • Understanding Patient Behavior
  • De-Escalation Skills: Verbal, Non-Verbal, Medicine Refusal, etc.
  • Role Play Scenarios 

INFECTION PREVENTION PROGRAMS

Facility Program: Train the Trainer 

Respiratory Protection/PPE Training

Staff will learn about the implementation of a Respiratory Protection Program, when respirators should be worn, doning and doffing PPE and the process of performing fit tests on staff and the selection of respirators

Staff Program: IP

IPC STARR

This program aims to establish an “IPC Champion” role at each facility to ensure there is oversight of IPC practices and staff training, targeted review and improvements of IPC policies and procedures, and administration support for IPC implementation. The program also offers a pathway for professional development in IPC for participating champions.

EMERGENCY MANAGEMENT

Staff Program: EM Office Hours

Emergency Management Office Hours

Each session focuses on a different emergency management topic and includes a breakout session for participants to discuss related thoughts and strategies with others in their region. Participants will submit attendance at the end of the session to receive CEs.

Staff Program: EM Tabletop Exercises & AAR

Emergency Management Tabletop Exercises & AAR

The Emergency Management (EM) Tabletop Exercise is a monthly event held on the last Tuesday of each month. Each session focuses on a different emergency management topic. If you're interested in an After Action Report, you can schedule a session in advance following the EM Tabletops. You must have attended 3 tabletop sessions before completing an AAR request.

LTC RISE 3.0 Emergency Management Offerings Guide is a comprehensive educational resource provided by HAPevolve. It includes 9 main video modules covering each aspect of the emergency management cycle, along with over 10 additional in-depth modules on hazard plans and tools like hazard vulnerability assessments.

Facility Program: PREP

PREP

The Portable Response Emergency Plan (PREP) is a web-based mobile app designed for health care organizations to host their emergency operations plans and facility-specific files.
