The End of LTC RISE, The Start of What's to Come!

 

"As of May 31st, LTC RISE has come to a close. This marks a significant transition for the South Central Pennsylvania long-term care community; one where the dedicated guidance, expert-led programming, and free resources that have supported your work may no longer be available in the same way.

However, the mission of LTC RISE does not end here.

The commitment to strengthening resident care, supporting staff, and building safer, more compassionate systems continues. The knowledge, tools, and frameworks gained from wound care to dementia care provide a lasting foundation for ongoing growth and improvement.

Even as LTC RISE concludes, opportunities remain to keep moving forward. Continued engagement with emerging programs, applying quality improvement strategies, and sustaining collaboration across teams will help carry this work into the future. The relationships built and the expertise developed established through RISE can continue to drive progress well beyond the program itself.

Together, this is not an ending but a continuation. By building on what has been accomplished, the long-term care community can continue advancing toward a future that is stronger, more responsive, and more resilient for both residents and staff. We are excited to continue to connect with your communities through our Penn State College of Medicine, Division of Geriatric Medicine opportunities in the future.

Dementia CareForce

Gain practical hands-on skills to navigate challenging behaviors presented in people living with dementia. This program focuses on understanding unmet needs, problem-solving tools, and utilizing huddles for enhanced communication.

Age-Friendly Health Systems (AFHS) Recognition

Resources will continue to be available through the Institute for Healthcare Improvement (IHI) website to learn more and get started with the AFHS process!

Long-Term Care RISE ECHO

While LTC RISE ECHO has concluded, we will have upcoming ECHO sessions for care partners for Persons Living with Dementia. 

COE-NF Behavioral Health Resources 

Established by the Substance Abuse and Mental Health Services Administration (SAMHSA), in partnership with the Centers for Medicare and Medicaid Services (CMS), the COE-NF offers Certified Medicare and Medicaid Nursing Facility Staff a centralized resource hub with easy access to trainings, technical assistance and additional resources, at no cost. Online webinars are still available.
