What is the purpose of the SCOR Survey? 

The survey is a nationally recognized, validated, and anonymous web-based tool designed to capture staff thoughts and perceptions on the culture impacting their engagement and well-being. It provides insights for long-term care (LTC) leaders to improve staff engagement, retention, and resident outcomes by comparing their interventions over time and against other LTC communities in Pennsylvania.

Who should participate in the survey? 

All long-term care communities are encouraged to participate, especially those who have previously taken the survey between May 2022 and June 2024. Those who participate will receive personalized results with recommendations. Communities that have experienced significant changes in leadership or staffing since their last survey should also consider participating to establish a new baseline.

When is the survey opportunity available?

Registration closes October 10th. The survey will be delivered from October 14 through November 11, 2024. Results available starting in December 2024.

5. What are the steps involved in completing the survey?

  1. Register: Sign up before October 10th.
  2. Mapping Call: the SCOR team will schedule a 15-minute call to determine the best way to display your results.
  3. Survey Preparation: LTC Leaders will promote the survey over approximately 2 weeks to their staff across different shifts.
  4. Pre-Launch Meeting: A 20-minute check-in with the SCOR team to ensure readiness.
  5. Survey Period: The survey is live for 4 weeks, with leaders receiving daily progress updates.
  6. Analytic Phase: While results are analyzed, we will schedule a 1-hour debriefing session.
  7. Debriefing Session: A 1-hour session to review results and gain actionable insights with the LTC Leaders of the facility and the SCOR team.
  8. Support: Optional monthly open office hours for 6 months to ask questions and gain support.
